Interlibrary Loan Request Form

Please read our Interlibrary Loan FAQ page and policy before submitting your request.

Beginning July 1, 2005: For any Interlibrary loan requests not picked up, a 50ยข fee will be added to your account. Please call the Interlibrary Loan office at (480) 644-2732 if you no longer need a request that you have submitted.
