Important Message:

This year, ONLY the Main location will carry ONLY the Federal notebook for photocopying of Federal tax forms. Forms at Main Library can be photocopied for 20-cents per page. Forms are also available for downloading on the Internet at the links below; in any library location, they can be downloaded and printed for 20-cents per page.

You will not be able to find tax forms loose/in bulk at any Mesa Public Library location. To locate loose paper forms, instructions, and booklets, you will need to contact the Arizona Department of Revenue and IRS (locations and links below).

FILING DEADLINE: Tuesday, April 17, 2011 (April 15th is a Sunday, April 16th is a federal holiday in the District of Columbia). Extension forms if needed: Federal: Form 4868. State: Form 204
